Can anybody that has one of these kits please post the breakdown of the contents by part numbers?

I'm specifically looking for the part number of the

Control Head Trunnion bracket

I believe the kit contains this part.

I will call Part ID in the morning, but if anybody can beat them to it, it would be appreciated.
